For your convenience, below are answers to some of our most frequently asked questions.
Arriving…Patients should arrive about 15 minutes prior to their appointment in order to allow time for any necessary paperwork. This allows us to offer prompt appointments that begin and end on time, benefiting all of our patients.
Health Issues…Concerns about health conditions such as pregnancy, impaired mobility, allergies or any other medical conditions will be discussed during the initial consultation.
Patient Comfort…Is very important to us and we do all that we can to ensure our patients are as comfortable as possible. We encourage our patients to please notify us if, at any time during their treatment, they are uncomfortable and we will do our best to make the necessary adjustments.Shhh…We request all cell phones be in quiet or vibrate mode. We also ask that small children not be brought to appointments, as this prevents disruptions. Minors under 18 must have a parent present to receive treatment.
Appointments & Cancellations…We request that patients schedule their appointment at least two weeks ahead of time. A $75 service fee will be charged for any appointments that are missed or canceled without 24-hour notice. All clients will be required to place a credit card on file before booking.

Retail…Unopened products may be returned within 30 days, with receipt, for credit only.
Consultations…Initial consultations are complimentary.
Financing…Available through the Advance Care Card. Apply at EvolvMD or on the Advance Care website.
